The occupational therapist will design an individual therapeutic plan in conjunction with the individual’s preferences and in partnership with the relevant trust professional and staff team. These will be facilitated on site and the development of opportunities to increase social inclusion through risk managing community integration to enable individuals to maintain relationships, develop wider social networks and enhance community connections. There will also be where appropriate to offer therapeutic interventions to maximise an individual’s opportunity for a healthy lifestyle and rehabilitation, these will form a key component in the monitoring of the individual’s health needs in collaboration with the relevant trust professional.
